Abstract: Healthcare blockchain systems are critical for managing patient data across distributed medical facilities, yet production deployment remains limited due to insufficient latency for emergency care, weak Byzantine fault tolerance in untrusted consortiums, and patient data exposure to validators violating privacy regulations. This paper presents the Dynamic Adaptive Sharding (DAS) Framework, the first integrated healthcare blockchain combining Zero-Knowledge Proofs (ZKPs), Reputation-Based Byzantine Fault Tolerant (RB-BFT) consensus achieving 66.7% fault tolerance, dynamic adaptive sharding with 92% load prediction accuracy, and Intelligent Service Organization (ISO) clinical context-aware routing. Through comprehensive simulation of 7,020 realistic healthcare transactions across 24 hours, we demonstrate significant improvements: 1.38×–1.56× latency improvement achieving 130ms average compared to 180ms baseline, 66.7% Byzantine tolerance providing 33% improvement over traditional BFT, 98.69% transaction success rate with zero patient data exposure, dynamic shard scaling from 5–31 shards preventing 90% of bottlenecks, and automatic HIPAA/GDPR compliance through cryptographic enforcement. The framework statistically validates these results with p < 0.0001 and large effect size (d =3.98), demonstrating feasibility for healthcare organizations with over 20M annual transactions. DAS Framework bridges a significant gap in healthcare interoperability by combining academic innovation in consensus mechanisms and privacy-preserving architecture with practical deployment of mission-critical blockchain systems and regulatory compliance frameworks.
